    Medical information for people arriving from Ukraine, who have temporary protection status in Bulgaria

    Ukrainian citizens with temporary protection in Bulgaria have equal rights to health care with Bulgarian citizens. Health insurance of Ukrainian citizens over 18 and under 63 for women and under 65 for men is covered by the state for a period of 90 days from the date of temporary protection. For persons under 18 years of age and over 63 years of age for women and over 65 years of age for men, the health insurance contributions will be at the expense of the state for the entire period of temporary protection. The amount of mandatory health insurance contributions for persons who are not registered as self-insured and who are not insured under an employment or civil contract is BGN 28.40 per month. These persons must submit a Form 7 declaration to the National Revenue Agency. The deadline for making these installments is the 25th of the month following the month to which it refers. If the payment is made late, interest is charged on the late payments. For example, the health contribution for the month of October 2022 must be paid by November 25, 2022 at the latest. Citizens’ health insurance rights, which are provided for your account, are interrupted if more than three long monthly installments have not been paid. Citizens’ interrupted health insurance rights are established on the condition that they pay all due health insurance contributions. Entitlements are recovered from the payment details of all health insurance contributions due for that period. Ukrainian citizens can use a reference on their health insurance status, as well as check the periods for which they lack health insurance contributions, through the NAP electronic service, which is freely accessible: https://portal.nra.bg/home.html# /goto:rep.HealthStatus.home The payment of health insurance can be made in the NRA service salons with debit and credit cards at POS terminals, by bank transfer, by postal order or electronically. When the obligation to pay insurance contributions rests with the employer or another person, failure to pay insurance contributions does not deprive the insured person of health insurance rights.

    Tuberculosis is a serious infectious disease that is transmitted from person to person by airborne droplets. It is caused by tuberculosis bacteria and mainly affects the respiratory system. Tuberculosis is a treatable disease. The treatment lasts a relatively long time – at least six months, and requires mandatory compliance with all recommendations of the doctor. Symptoms include: cough lasting more than 2 weeks, chest pain, sputum production, weakness or easy tiredness, weight loss, loss of appetite, fever, night sweats.

    The infection is transmitted by patients with pulmonary tuberculosis who secrete tuberculosis bacteria. People in close contact can become infected by breathing air containing these bacteria, sneezing, coughing or sputum from the patient. Particularly vulnerable to the close environment of the patients are children, the elderly over 65 and people with impaired defenses of the body (chronic stress, malnutrition, alcohol and drug addiction, HIV infection, concomitant chronic diseases and others.

    There is a risk of disease in close contact with a person with tuberculosis of the respiratory tract, if you are not vaccinated against tuberculosis, spend a long time indoors with many people, the body’s resistance is weakened, you have alcohol and drug addiction, concomitant chronic diseases , you are a carrier of the HIV virus.

    In crisis situations, these risks increase.

    Hepatitis is a disease caused by a virus. Of the five different hepatitis viruses (A, B, C, D and E), only B and C can become chronic, causing prolonged illness and severe liver damage. An expert estimate of 15 million people in the World Health Organization (WHO) European Region is living with a chronic hepatitis B infection and 14 million are infected with hepatitis C. The main routes of transmission of these infections are three: sexual, blood and vertical (from the mother to her child).     Vaccination

    Vaccines against 11 acute infectious diseases of personal and public health are administered in Bulgaria. These include tuberculosis, diphtheria, tetanus, pertussis, polio, hepatitis B, haemophilus influenzae type B, pneumococci, measles, mumps and rubella.

    Immunizations against these diseases are mandatory for all children in the country, and vaccines are administered according to the National Immunization Calendar.

    Mandatory immunisations in the Republic of Bulgaria are similar to the immunizations applied in the Republic of Ukraine.

    However, there are a few main differences:

    • Immunisation against pneumococci is not applied in Ukraine;
    • In Ukraine, live and killed polio vaccine is used, and in Bulgaria – only killed polio vaccine;
    • In Ukraine, fewer pertussis vaccine doses are used;
    • Fewer doses of tetanus and diphtheria vaccine are used in Ukraine;
    • In Ukraine, fewer hemophilus influenza vaccines are used.

    These differences, as well as the fact that in recent years the immunization coverage against the listed diseases in both countries is below the required level of 95% to ensure that there will be no outbreaks and epidemics of vaccine-preventable infectious diseases, necessitates all children with missing data or with incomplete immunization status to be vaccinated.

    For this purpose, a doctor from the regional health inspectorate in the area where you and your child will reside or the general practitioner you choose for your child will prepare an individual immunization plan for the necessary immunizations by type of vaccine and number of doses. For this purpose it is necessary to bring all available medical documents certifying the vaccines given to your child so far.

    Every child from Ukraine who has received temporary protection in the Republic of Bulgaria, certified by the received Personal Number of a foreigner, has the right to immunizations.

    Immunizations are also needed to visit your child in a nursery, kindergarten or school, without which this is impossible.

    Interim guidelines for immunisations and re-immunisations of children arriving from Ukraine

    A child with full immunisation status for Ukraine 

    • up to 5 years of age – 2 doses of Synflorix (minimum interval between 2 months)
    • over 5 years of age – Synflorix is not required

    Child with incomplete immunisation status for Ukrainethe missing doses of vaccines are applied, according to the Immunisation Calendar of Bulgaria

    • up to 1 year of age
      – 1 to 3 doses of Pentaxim, according to available data on vaccines against diphtheria, tetanus, pertussis, polio, haemophilus influenzae; 
      – 2 doses of Synflorix (minimum interval between doses of 2 months); 
      – Mantoux test /BCG
    • between 1-4 years
      – 1 dose of Pentaxim + 2 doses of Tetraxim (basic immunisation) – according to the available data for vaccines against diphtheria, tetanus, pertussis, polio, haemophilus influenzae;
      – 2 doses of Synflorix (minimum interval between doses of 2 months);
      – 1 dose of M-M-RVAXPRO;
      – Mantoux test /BCG;
      – reimmunisation with 1 dose of Pentaxim (in children under 4 years)
      – reimmunisation with 1 dose of Tetraxim (in children aged 4 years and over).
    • between 5-6 years 
      – 1 to 3 doses of Tetraxim;
      – 1 dose of M-M-RVAXPRO;
      – Mantoux test /BCG.
    • between 7-13 years 
      – 1 dose Tetraxim (basic immunisation); 
      – 1 dose of Td (basic immunisation); 
      – 1 dose of M-M-RVAXPRO;
      – Mantoux test /BCG; 
      – 1 dose of Tetraxim (reimmunisation). 
    • over 13 years
      – 1 dose of M-M-RVAXPRO;
      – 1-2 doses of Td (basic immunisation); 
      – Mantoux test /BCG; 
      – 1 dose of Boostrix (reimmunisation). 
       

    Child without data on immunizations – considered unvaccinated. It is covered by immunisations, according to the Immunisation Calendar of Bulgaria, taking into account the following:

    • children under 1 year of age – apply: 3 doses of Pentaxim, 2 doses of Synflorix, Mantoux test /BCG. 
    • children aged 1-4 years – apply: 1 dose of Pentaxim + 2 doses of Tetraxim, 1 dose of M-M-RVAXPRO (priority coverage with M-M-RVAXPRO over Synflorix), 2 doses of Synflorix, Mantoux test /BCG.
    • children 5-6 years of age – apply: 3 doses of Tetraxim, 1 dose of M-M-RVAXPRO, Mantoux test /BCG.
    • children aged 7-13 years – apply: 1 dose Tetraxim, 1 dose Td, 1 dose M-M-RVAXPRO, Mantoux test /BCG.
    • children over 13 years of age – apply: 2 doses Td, 1 dose M-M-RVAXPRO, Mantoux test /BCG.

    Information for pregnant women

    Currently, pregnant women from Ukraine arriving in Bulgaria can be examined at the Health Consulting Centers opened at 31 medical institutions in the country, as well as at hospitals that have announced open doors for Ukrainian citizens.

    Those of them who register for temporary protection are also entitled to free hospital treatment for them, if necessary.

    Emergency care is also free, and if necessary you can call an ambulance on 112.

    The rights of pregnant women, as well as representatives of other vulnerable groups under temporary protection, will be equated with those of health insured Bulgarian citizens with legal changes, which are expected to be considered soon by the National Assembly.

    Can Ukrainian doctors and nurses work in Bulgaria?

    In order to acquire the right to practice their profession in Bulgaria, citizens of Ukraine who have acquired the professional qualification “doctor” or “nurse” in Ukraine must successfully pass an examination in Bulgarian language and professional terminology in Bulgarian, as well as to pass an examination, including the state examinations, determined in the uniform state requirements for acquiring the respective medical profession in Bulgaria (examination under art. 186, para 3, item 3, letter “a” of the Health Act).

    The exam in Bulgarian language and professional terminology in Bulgarian language is organized and conducted by the Center for Assessment in Preschool and School Education (COPUO) at the Ministry of Education and Science.     There are no legal requirements regarding where and how to conduct training in Bulgarian. It is only necessary at the end for the applicant to pass the exam in Bulgarian language and professional terminology in Bulgarian language in COPUO.

    In order to take the exam, which includes the state exams, defined in the uniform state requirements for acquiring the respective medical profession in Bulgaria, it is necessary to pass a procedure for admission to this exam. The procedure is organized and conducted by the Ministry of Health, is free of charge for applicants and generally involves the submission of an application accompanied by certain documents (specified in the application).     As a result of the procedure, the Minister of Health issues an order for admission to an exam. On the basis of this order, the applicant takes the exam in a higher school of his choice, which provides training in the profession. The higher school has the right to charge a fee for taking this exam. It issues a certificate for the successful passing of the exam, which gives the right to practice the relevant medical profession in Bulgaria.
